1. Coach / Team Contact Info

• Provide name, email address, and phone number of head coach or team manager.

2. Roster Upload

• Submit a complete roster with full names and ages of all players.

• Rosters must meet the minimum (11)  and maximum (14) player requirements.

• Ensure every player fits the age eligibility for their division.

• Due Date for Rosters: March 22, 2026

• Email to: info@flatlandyouthfootball.com or upload directly in this registration.

3. Heads Up Concussion Waiver

• Required for every athlete on the team.

• Coaches must ensure all players and guardians review and agree to concussion safety protocols.

4. Checkout & Payment

• Proceed through checkout to complete registration.

• Payment of $350 is required to finalize and confirm team participation.
